Pear Suite The health program is eager to sign with community health workers (CHW) to close the care gap and address the social driving force of health. However, CHWs are often based on community organizations, lacking the tools and expertise to become an in-network provider, safely sharing health care data and filing claims, preventing these partnerships. To fully participate in hard-to-reach Medicare, Medicare and D-SNP members, the company believes that the turnkey solution is needed in partnership with CHWS. With an all-in-one platform, community organizations can now provide members with SDOH screenings, health education, navigation and resource recommendations in a sustainable, people-centered way. Its network of providers helps health programs build network capabilities while addressing care gaps at scale.
